N2 - Since 2017, Amsterdam UMC's Nephrology outpatient clinic has been working on value-driven care for chronic kidney failure. The aim is to ensure that the care better meets the preferences and needs of the patient. On the one hand, it would be done by allowing the patient to participate in the discussion about improvements in care, while also using outcomes and shared decision-making in the consultation room. By removing existing barriers between departments, we are improving care at the Nephrology outpatient clinic together with our colleagues and the patient.
