Successful fibrinolytic therapy for massive pulmonary embolism during pregnancy

Abstract

A case of massive pulmonary embolism with shock during the 27th week of pregnancy was treated successfully with streptokinase infusion into the pulmonary artery, mechanical ventilation and circulatory support. Lysis of the emboli was monitored by the difference between arterial and end-tidal PCO2. Although the fibrinolytic therapy induced bleeding in the mother, it caused no harm to the foetus which was delivered successfully at full-term.
